Abstract•I comment on the scientific contributions of the immunologist, Nelson Monteiro Vaz, with... more Abstract•I comment on the scientific contributions of the immunologist, Nelson Monteiro Vaz, with whom I have been working over the last 20 years. Inspired by the work of Maturana, Vaz has spent 50 years in an ongoing research effort towards what can be considered a paradigm shift in immunology. This has repercussions on how we understand physiology and pathophysiology, nutrition and diet, the microbiome and the very way we explain health and disease. The paradigmatic change is grounded in Maturana’s biology of cognition and evolution through natural drift, and demonstrates the wide-reaching implications of the premises under which we do science by noting how these have consequences for public health and medical disease theories.

Inspirado pelo trabalho de Maturana, Vaz passou 50 anos em um esforço contínuo de pesquisa para o que pode ser considerado uma mudança de paradigma na imunologia. Isso tem repercussões na forma como entendemos a fisiologia e a fisiopatologia, a nutrição e a dieta, o microbioma e a própria maneira como explicamos a saúde e a doença. A mudança paradigmática é fundamentada na biologia da cognição e evolução por meio da deriva natural de Maturana e demonstra as implicações de amplo alcance das premissas sob as quais fazemos ciência, observando como elas têm consequências para a saúde pública e teorias de doenças médicas.

The patient is a 60-year-old female with functional class III/ IV (NYHA) heart failure of an immu... more The patient is a 60-year-old female with functional class III/ IV (NYHA) heart failure of an immunomediated etiology (autoim-mune myocarditis). After unsuccessful therapeutic attempts, intravenous immunoglobulin was used, leading to deterioration in renal function, a rare complication of that therapy. After hemodialysis, the patient’s renal function was restored, and the chronic heart failure improved to functional class I. Myocardites constitute a group of diseases characterized by immuno-inflammatory aggression against the myocardium. The anatomicopathological substrate of this aggression has been clas-sically described as a cellular infiltration of lymphocytes, macro-phages, and other leukocytes, with destruction of cardiomyocytes, myocytolysis (Dallas criteria)1. Like our last contribution, the paper that follows also references Shakespeare – but in a way that could hardly be more different.
Here, Professor Paul Heritage analyses different aspects of Hamlet and he does so with the help of Vitor Pordeus who is a medical doctor, psychiatrist, pub- lic health official, scientist, teacher, director and therapist.
But Heritage goes beyond Shakespeare, extending his exploration of madness and its treatment to Vitor Pordeus’s own experience both in his work at the theatre and treating patients in a clinical setting.
From the initial question it poses to the final reply, we learn both how the Shakespearean theatre has found its place on the psychiatric ward: how ‘sombre ancestral shadows’ may well hang over both the characters played by the actors and the actors themselves. Might similar, albeit not identical ancestral shadows, hang over us all?

A negative relationship between latitude and infectious disease species richness has been suggested. To examine whether their prevalence differs in two latitudinally different populations. The prevalence of infections with Toxoplasma gondii, rubella virus, cytomegalovirus, Epstein-Barr virus and Treponema pallidum was compared between subjects from Italy and Colombia. We found high titers of antibodies against four of five microorganisms tested, Toxoplasma gondii (50.8%), rubella virus (German measles) (75%), cytomegalovirus (86.3%), Epstein-Barr virus (83.3%) and Treponema pallidum (6.3%) in completely healthy individuals from a tropical country (Colombia) and a European country (Italy). The field of epigenetics has arisen as one of the most important scientific axis to understand development, physiology and pathophysiology in all areas of medicine with special impact in psychiatry a field that has been historically dominated by today obsolete biological models and theories. In this short review I try to reflect upon the impacts these new understandings about nature may help us to approach and threat psychic diseases.

We classified the factors as genetic, immune, hormonal, and environmental. For a specific autoim-mune condition to develop, a particular combination of these factors (like the pebbles or small pieces of glass or ceramics that comprise a mosaic) is required. Randomly generated and activated at random, these lymphocytes follow only their individual, clonal history. Thus, in traditional descriptions, immunological activity is neither systemic nor historical and is never “physiological”. However, recent descriptions show an abundant “auto”-reactivity in healthy organisms, an evidence of internal connectivity. The two major sources of immunogenic contacts, namely, dietary proteins and products of the autochthonous microbiota fail to induce progressive “secondary-type” clonal expansions (or “memory”). Natural IgM may arise in “antigen-free” organisms as they do in conventionally raised animals; actually, clonal receptors of both T and B lymphocytes are formed in antigen-free intracellular environments and are not driven by antigen exposure. A negative relationship between latitude and infectious disease species richness has been suggested. OBJECTIVES To examine whether their prevalence differs in two latitudinally different populations. METHODS The prevalence of infections with Toxoplasma gondii, rubella virus, cytomegalovirus, Epstein-Barr virus and Treponema pallidum was compared between subjects from Italy and Colombia. RESULTS We found high titers of antibodies against four of five microorganisms tested, Toxoplasma gondii (50.8%), rubella virus (German measles) (75%), cytomegalovirus (86.3%), Epstein-Barr virus (83.3%) and Treponema pallidum (6.3%) in completely healthy individuals from a tropical country (Colombia) and a European country (Italy). Com o intuito de transformar tal realidade foi criado o Nucleo de Cultura, Ciencia e Saude na Secretaria Municipal de Saude e Defesa Civil do Rio de Janeiro. Através do estudo da história da ciência, podemos precisar pontos de inflexão conceitual em que esta ideia do corpo-máqui-na foi introduzida e praticada sistematicamente até ganhar praticamen-te todas as mentes do mundo, que hoje entendem ser a própria ciência a fonte última de informações sobre nós próprios e a natureza. O ponto de fundação da ciência moderna é precisamente a obra de René Descartes (1485-1650). E, como toda doença traz em si seu antídoto, é no próprio século XVII, também na Holanda, onde Descartes viveu e trabalhou, que encontraremos a síntese paradigmática alternativa que nos salva-rá da cilada do corpo-máquina: a Ética, de Baruch Spinoza (1632-1677). Spinoza explica de forma científica e testável os principais enigmas con-ceituais que persistiram desafiando a ciência moderna: Deus, Afetos/ Corpo, Mente, Escravidão mental e Liberdade mental. Em tempos de pandemias infecciosas e de doenças mentais/desordens do pensamento, esses temas impõem-se de forma inequívoca: até que o novo paradigma adequado seja amadurecido, compreendido e praticado, a humanidade e o meio ambiente, precisamente os sistemas biológicos, continuarão pa-decendo de sofrimento psíquico/ecológico. Abstract The psychiatrist Nise da Silveira (1905-1999) pioneered several major innovations in Brazilian mental health policy. She developed a clinical practice grounded in the principles of Jung's analytical psychology and applied this approach to the spontaneous expressive productions of patients with chronic psychosis. In 1946, she created the Museum of Images of the Unconscious, which maintains an on growing archive of some 360,000-art works by patients of the oldest Brazilian psychiatric hospital. She also inaugurated, the House of the Palms (Casa das Palmeiras) in 1957, one of the first outpatient cultural clinics. She published hundreds of articles and several books, the most important of which include: Images of the Unconscious, The World of Images, Letters to Spinoza, Jung: life and work, and Cats: Emotion of Coping. She also produced dozens of film documentaries, three of them in collaboration with the Brazilian filmmaker Leon Hirszman and organized hundreds of art exhibitions. She trained generations of psychiatrists, occupational therapists, and psychologists, and influenced many artists and people from all walks of life. Working at a time when psychiatric therapeutics was dominated by Electroconvulsotherapy-ECT and lobotomy, she fought for a paradigm change that was rooted in political recognition of patients' agency and respect for the self-healing functions of art and the unconscious. Her work remains a vital 2 inspiration for a new generation of mental health workers in Brazil and in the world.

Resumo Os traumas que constituem entrave ao desenvolvimento do Brasil têm sua origem, segundo o n... more Resumo Os traumas que constituem entrave ao desenvolvimento do Brasil têm sua origem, segundo o nosso diagnóstico, na baixa autoestima e na fragmentação de nossa autoimagem, típicas de povo colonizado e provocadas pelos imensos traumas históricos e transgeracionais, nomeadamente: a Inquisição e os genocídios de indígenas e de afro-brasileiros ainda em curso. O brasileiro descende do encontro entre povos oprimidos e elites opressoras, situação que produz esquizofrenia social e fragmentação da mente coletiva. O resultado foi uma sociedade economicamente rica, mas socialmente muito pobre, com a maior taxa absoluta de homicídios do planeta, alta mortalidade infantil, índices de educação ainda medievais, e a falência histórica da construção de uma cultura de igualdade, cooperação e de aceitação das próprias origens, pré-requisito para um regime democrático próspero com liberdade de expressão.

Abstract: We face a scientific and political historical moment of mental health crisis worldwide.... more Abstract: We face a scientific and political historical moment of mental health crisis worldwide. It is a clear that the current paradigm of modern biology, namely Neo-Darwinism and its derivatives in medicine and public health, show alarming signs of exhaustion and iatrogenic damage to human and biological health. In no other field this contradiction becomes so blatant as in public mental health. We are witnessing the growing of a pandemic of mental illness that the own scientific system managed to construct and lose control. The excessive, widespread, worldwide use of psychotropic pharmaceuticals, the alarming reports of iatrogenic side effects and deaths, the growing levels of violence, genocides, wars in virtually every part of the planet denounces to us that the dominant scientific biomedical model are informing us wrong. In general, it is the scientific model that informs government decisions that induce the population to ultimately act with violence and rupture of biological relationships; from which our existence depends. A new biological paradigmatic synthesis has been published and developed in the last decades. This stems from practically observed experimental phenomena arising virtually from all fields of biology and medicine, specially, from socio-economic determination of mental health, when studies that show the major impacts of environment, education and level of income. In this paper, I shall present a comprehensive model of human biology and psychopathology rooted in the pioneering scientific works of the neurobiologist Humberto Maturana and the psychiatrist Carl Jung illustrating with contemporary scientific documented experiences of the American psychiatrist John Weir Perry and the Brazilian woman psychiatrist Nise da Silveira in the field of mental health promotion and our own documented and published experience in mental health promotion working under the new paradigm, in order to make sense of those apparent contradictions, and heal our ideas and practices.

Sinopse: Documentário produzido por Raphael Peixoto, Tarcila Viana e Henrique Faerman, que procura entender a forma com que os clientes vem sendo medicados no Instituto Municipal Nise da Silveira. Com doses diárias de afeto (catalisador), pílulas de música, descargas de teatro e injeções de compaixão, os ocupantes dos 3° e 4° andar já caminham livremente (livre-mente), já dialogam e olham nos olhos. A rotina já não é tão medicamentosa. Ócio entre essas paredes coloridas? Só por opção. O objetivo é transmitir o cotidiano dos que circulam o número 521 da Ramiro Magalhães no Engenho de Dentro e constatar que a arte, em suas mais diversas formas, é uma excelente ferramenta de cura seja ela espiritual, mental ou social.

Art plays a unique role in human experience both as an individual and a social mode of expression and communal activity. All societies have traditions of fashioning objects, language, and performance in ways that serve to transmit culture, explore the world, entertain, and edify. Active engagement with the arts can transform suffering, give meaning to af iction, and support recovery. This conference will bring together artists, scholars, researchers, and professionals involved in mental health, to discuss the role of the arts in cultural psychiatry. Art can be used to build and express individual and collective identity, as a creative process that yields new ways of experiencing the world, as a social and political intervention to critique or challenge existing frameworks, and as a modality for therapeutic interventions. Sessions will explore topics related to several broad themes:
• the nature of creative artistic and aesthetic processes of invention, enactment and improvisation;
• the role of the arts in constructing and expressing individual and collective identities—especially, public, social or political uses of art to raise awareness and challenge marginality and oppression;

• the arts as media for articulating, understanding and coping with the experience of mental health and illness;
• art making as a creative medium
for individual therapeutic exploration, growth, and transformation and for collective con ict resolution and mental health promotion.
The format will be a one-day conference (June 1) for mental health professionals, social scientists, and the general public, and an intensive two-day workshop (June 2-3) for researchers working in these areas. Workshop papers will be pre-circulated to participants so that the emphasis will be on intensive discussion
