You play as Phlegmwad, a disfigured creature whose face is hidden by a Brown Bag Mask. Phlegmwad is an assassin serving the terrifying Lord Horrible, who assigns him on a task to kill the beautiful Queen of Everything. Will he complete his mission? Will he switch sides? Will he save the day? It's all up to you!

The game provides examples of:
- Heel–Face Turn: One story option is to switch sides and team up with the Queen of Everything and even romance her.
- Love Potion: You can buy one at the flea market. Interestingly, you can also buy a hate potion from the same vendor.
- Scatterbrained Senior: Once you arrive to the Great Wizard to ask him where is the Sword of All That is Good, you find out that he has gone senile — so you need to find out about the Sword in another way.
- Shout-Out: In the episode when Phlegmwad meets Dietrich Squinkifer, there's a poster with Sam & Max hanging on the wall (see illustration above).
- Villain Protagonist: You start out as one, being an assassin on Lord Horrible's service. But it depends on your playthrough whether you will be a villain, a hero, or something in-between.
